Fern Support Coordination

We offer support coordination for participants under the National Disability Insurance Scheme (NDIS), and can deliver services through face-to-face meetings, phone calls, video meetings, or written communication such as text or email. Support coordinators play a crucial role in building participants' capacity to self-manage their NDIS plans over time. This includes providing education and training on topics such as budgeting, decision-making, and advocacy, empowering individuals to take greater control over their lives and support networks. We offer flexible arrangements, with appointments available to be flexible after hours and on weekends. Fern Support is proud to deliver services with both clinical and lived experiences as a fellow NDIS participant! We understand the ins and outs, and can emphasise and apply lived experiences backed with tertiary education and ongoing training and development within Australia. We are an inclusive practice, welcoming individuals from all walks of life. We work closely with young adults, LGBTQI+ community, families, caregivers, culturally and linguistically diverse (CALD) individuals and more to ensure that our services meet the needs of everyone under our wing. Our mission is to provide compassionate and personalized support coordination. We focus on quality over quantity, and therefore we provide the utmost care.
