They are there to help you to make the best use of your supports in your NDIS plan. At LCC Support Service, a Support Coordinator can assist you in the following areas- Understand and use your NDIS plan to pursue your goals. Connect you with NDIS providers, community, mainstream and other government services that meet your individual needs. Build your confidence and skills to use and coordinate your supports. Help you to direct the NDIS through the different phases of your NDIS journey. Find quotes for services and establish Service Agreements with providers. Link you into (and navigate) employment or educational settings and community activities. Coordinate service providers and supports. Organise assessments and reports through allied health specialists and other medical services. Help find suitable accommodation and respite options. Monitor your NDIS plan outcomes and expenditure, and report back to the NDIA. Preplanning and re-developing your goals prior to the review of your plan.
